ENHANCING POSTEROLATERAL SPINAL FUSION WITH rhBMP-6 EXPOSED OSTEOPROGENITORCELLS

چکیده
INTRODUCTION: The nonunion rate after posterolateral fusion has been reported to be as high as thirty-five percent, even with the use of autogenous iliac crest bone graft (ICBG) [4]. Accordingly, alternative techniques for expanding or stimulating the spinal fusion process are actively being investigated [1]. For the past several years we have been developing a strategy for stimulating marrow-derived osteoprogenitor (OP) cells by exposure to a cell-derived osteoinductive matrix containing rhBMP-6. Our preliminary data suggests that this exposure induces osteoblastic differentiation [4] and that the induced cells are capable of increasing new bone formation in the New Zealand white rabbit (NZW) posterolateral intertransverse fusion model [6]. In this study we have expanded our prior work [6] to assess the osteogenic capacity of two different concentrations of rhBMP-6 exposed osteoprogenitor cells.

STUDY DESIGN Single center retrospective cohort analysis. PURPOSE The goal was to evaluate the influence of varying amount of recombinant human bone morphogenetic protein 2 (rhBMP-2) per level on fusion rates and complications in posterolateral spinal fusions. OVERVIEW OF LITERATURE rhBMP-2 has been utilized for lumbar posterolateral fusions for many years. Initial rhBMP-2 recommendations w...

Study Design Randomized, controlled animal study. Objective Recombinant human bone morphogenetic protein-2 (rhBMP-2) is frequently utilized as a bone graft substitute in spinal fusions to overcome the difficult healing environment in patients with osteoporosis. However, the effects of estrogen deficiency and poor bone quality on rhBMP-2 efficacy are unknown. This study sought to determine wheth...
متن کاملInjectable and compression-resistant low-viscosity polymer/ceramic composite carriers for rhBMP-2 in a rabbit model of posterolateral fusion: a pilot study
BACKGROUND The challenging biological and mechanical environment of posterolateral fusion (PLF) requires a carrier that spans the transverse processes and resists the compressive forces of the posterior musculature. The less traumatic posterolateral approach enabled by minimally invasive surgical techniques has prompted investigations into alternative rhBMP-2 carriers that are injectable, setta...

متن کاملPosterolateral inter-transverse lumbar fusion in a mouse model
BACKGROUND Spinal fusion is a common orthopaedic procedure that has been previously modeled using canine, lapine, and rodent subjects. Despite the increasing availability of genetically modified mouse strains, murine models have only been infrequently described. PURPOSE To present an efficient and minimally traumatic procedure for achieving spinal fusion in a mouse model and determine the opt...
